SUPPLEMENTAL INFORMATION

Proposal for the following collection of information:

Title: Application of a State Chartered Credit Union for Insurance of Accounts.

OMB Number: 31330011.

Form Number: NCUA 9600.

Type of Review: Reinstatement, without change, of a previously approved collection.

Description: Section 201 of the Federal Credit Union Act (12 U.S.C. 1781) requires statechartered credit unions desiring federal insurance to submit an application. The requirement also applies to federal credit unions converting to state charters and desiring federal insurance.

Respondents: State chartered credit unions and federal credit unions converting to state charter that desire federal insurance of member accounts.

Estimated No. of Respondents/Recordkeepers: 15.

Estimated Burden Hours per Response: 3.6 hours.

Frequency of Response: Reporting. Upon application for federal insurance.

Estimated Total Annual Burden Hours: 54 hours.

Estimated Total Annual Cost: N/A.
