Title: Application for Reinstatement, VA Form 29352 (Insurance
Lapsed for more than 6 months) and VA Form 29353 (Nonmedical Comparative Health Statement).
OMB Control Number: 29000011.
Type of Review: Extension of a currently approved collection.
Abstract: VA Forms 29352 and 29353 are used to apply for reinstatement of insurance and/or Total Disability Income Provision that has lapsed for more than six months. VA uses the information collected to establish the applicant's eligibility for reinstatement.
An agency may not conduct or sponsor, and a person is not required to respond to a collection of information unless it displays a currently valid OMB control number. The Federal Register Notice with a 60day comment period soliciting comments on this collection of information was published on September 25, 2007 at pages 5451254513.
