Sunshine Act Meeting--September 24, 2009--6:30 pm ]]></title>

</p><p>    In connection with its investigation into the cause of a February 
7, 2008, an explosion and fire at the Imperial Sugar refinery northwest 
of Savannah, Georgia, the Chemical Safety and Hazard Investigation 
Board announces that it will convene a public meeting on September 24, 
2009, starting at 6:30 pm at the Hilton Savannah DeSoto15 East  Liberty Street, Savannah, Georgia.</p><p>    At the meeting CSB staff will present to the Board the results of 
their investigation into this incident. Key issues involved in the 
investigation concern combustible dust hazard recognition, minimizing 
combustible dust accumulation in the workplace, and equipment design 
and maintenance. This will be followed by a public comment period prior  to a Board vote on the report.</p><p>    Incident: On February 7, 2008, at about 7:15 p.m., a series of 
sugar dust explosions at the Imperial Sugar manufacturing facility in 
Port Wentworth, Georgia, resulted in 14 worker fatalities and 36

Sunshine Act Meeting--September 15, 2009--6 p.m. ]]></title>

</p><p>    In connection with its investigation into the cause of a December 
19, 2007, explosion and subsequent chemical fire at the T2 
Laboratories, Inc., a chemical manufacturer in Jacksonville, Florida, 
the Chemical Safety and Hazard Investigation Board announces that it 
will convene a public meeting on September 15, 2009, starting at 6 p.m. 
at the Marriott Hotel located at 4670 Salisbury Rd in Jacksonville,  Florida in ``The Florida Room.''</p><p>    At the meeting CSB staff will present to the Board the results of 
their investigation into this incident. Key issues involved in the 
investigation concern reactive hazard recognition, hazard education, 
emergency preparedness, and process design and scaleup. This will be 
followed by a public comment period prior to a Board vote on the  report.</p><p>    Incident Description: On December 19th, T2 Laboratories, Inc. was 
producing a batch of methylcyclopentadienyl manganese tricarbonyl 
(MCMT). A problem soon occurred and the process operator had an outside

  <title><![CDATA[
Chemical Release Reporting ]]></title>

  <description><![CDATA[ The Clean Air Act requires that the Chemical Safety and Hazard 
Investigation Board (CSB) establish a regulation which would require 
that accidental chemical releases be reported to the CSB or to the 
National Response Center. With this advance notice of proposed 
rulemaking, the CSB seeks to obtain comments on how best to proceed 
with implementing this requirement. The CSB will use this information 
in the development of a proposed and then a final rule.

  <title><![CDATA[
Sunshine Act Meeting ]]></title>

</p><p>    In connection with its investigation into an explosion and fire 
that occurred at the Bayer CropScience facility in Institute, West 
Virginia, on August 28, 2008, the U.S. Chemical Safety Board (CSB) 
announces that it will hold a public meeting on April 23, 2009, in 
Institute, West Virginia, to present preliminary findings from its 
investigation of the explosion that fatally injured two workers.
</p><p>    The meeting will begin at 6:30 p.m. at the West Virginia State 
University Wilson Building, Multipurpose Room, 103 University Union, 
Institute, WV 25112. The meeting is free and open to the public. Pre
registration is not required, but to assure adequate seating, attendees  are encouraged to preregister by emailing their names and <br/>
affiliations to <A HREF="mailto:bayer@csb.gov">bayer@csb.gov</A> by April 10.
</p><p>    At the meeting, the CSB investigative team will present its 
preliminary findings on the circumstances of the accident to the CSB 
Board and the public. The Board will ask questions of the team in<script type="text/javascript">
